Maternal and Child Health Handbook / Prenatal Baby Medical Check-Up Tickets
Residents from other cities who took refuge in Sendai City can apply for a Maternal
and Child Health Handbook and Prenatal & Baby Medical Check-Up Tickets
at any Ward Office or Branch Office
Tax
Municipal tax declaration and return etc. procedures will be postponed until
further notice. Please check the Municipal Tax Postponement Notice for more
details.
Registration procedures for motor cycles with engines of less than 50cc displacement
will take place as usual at the Taxation Section of each Ward Office and General
Branch Office as well as the Tax and Resident Affairs Section of Akiu General
Branch Office.
If your vehicle was damaged or missing due to the earthquake and tsunami, you
can apply for a municipal tax declaration which will be processed at each Ward
Office and General Branch Office as well as the Tax and Resident Affairs Section
of Akiu General Branch Office.
National Health Insurance
According to the notification of the government, even when you canft present
the insurance card to the medical institution by losing it, you still can consult
a physician by stating your name, date of birth, and address.
The insurance
payer who suffered from the earthquake devastation can be applied to the delay
and exemption measurement for partial medical fees at the medical institution
until May if they are under the conditions such as all or partial destructions
of their houses, deaths, or serious wounds. Please apply for the exemption at
the hospital, the drugstore, and the home-visit nursing care station.
If
those who are eligible for the delay and exemption mentioned above have already
partial medical fees, they can be paid back by application. So please inquire.The
objects of the refund will be partial medical fees to the health insurance treatment
and the meal fees/the life medical expenses (amount of a standard load) in hospitals.

Public Nursing Care Insurance
Even when you canft present the long term care insurance card to the long term
care insurance service entrepreneur by having lost it, you are able to receive
the long term care insurance service by stating your name, date of birth, and
address.
The certification for long-term care application is accepted in a ward office
and an integrated branch office.The visit investigation will be corresponded
to the one who requires emergency by priority. After the visit investigation
and committee for recognition for the long term care insurance become in order,
the recognition result notification will be reported.
There is a system that reduces the userfs load of the nursing care premium and
the long term care insurance service according to the ratio of the damage if
the insurant or subsistence person's property received remarkable damage due
to the disaster. The application is accepted in a ward office and an integrated
branch office. When applying, you may have to submit a Disaster Victim Certificate.
After the reduction application of the userfs load of the long term care insurance
service is completed in an integrated branch office, the reduction recognition
certificate will be later sent from the ward office.

Medical Subsidy for Infants
We give advice and accept applications for a health care subsidy for infants/people
with disabilities/children with a single parent, child care benefit, a child-raising
allowance, and welfare loans for a single-mother household/a widow.
If you have misplaced your Health Care Subsidy for Infants Certificate, you
can receive a replacement.
For those people who have lost their recipient certificate of a health care
subsidy for infants, please note that it is still possible for infants to receive
medical treatments with no costs. Please advise to the staff at the hospital
accordingly.

Child Vaccinations
BCG Vaccinations
BCG vaccinations to help prevent tuberculosisis were cancelled
for March. Vaccination dates for the rest of the year have yet to be decided.
Polio Vaccinations
Vaccinations for April have been cancelled due to the
earthquake. If you missed out on receiving a vaccination in April, the next
vaccination period will take place in autumn.
